company name
correspondence address
e-mail address
contact person and contact telephone number
any other relevant information
You are also advised to change the authorised signer of your company resulting from staff movement.
What to Do
Complete the Information Update Form (For Participating Employer) to change general information.
Complete the Authorised Signature Specimen Form to change your company's authorised signer.
Please send the original forms to us within 30 days after the effective date of the change.
Lost or Forgot the PIN of Your Company Account?
Simply complete the PIN Re-generation Request Form and return the original completed form to us. To ensure security and privacy, a new PIN will then be sent directly to the contact person of your company by mail.
