Update on member enrollment forms

                                                              The clause under the part of "Authorisation, Declaration and Consent" which authorises employers, banks and trustees, etc to disclose information to BCT in circumstances necessary has been deleted from the member enrollment forms below to align with our new practices. For members having used the previous version of the forms, please be advised that the authorisation given under the clause has been treated as abolished (ie as though it has never been given).

                                                              • BCT (MPF) Industry Choice - Member Enrollment Form (Ver.15a-042015), Personal Account Member (Ver.15a-042015) and Casual Employee Application Form (Ver.12a-042015)

                                                              Enrollment Timeline: within 60 days of employment.

                                                              Download and complete the Member Enrollment Form (Note: Authorised signature with company chop are required) and send it to us.

                                                              Upon completion of your enrollment
                                                              • Notice of Participation will be sent to your employees within five working days upon receipt of the enrollment form.
                                                              • Email or SMS notification will be sent to your employees for retrieving a verification code, which could be used for password set up via member website afterwards.

                                                              Enrollment Timeline: the next working day following the 1st pay day or the 10th day from the commencement of employment, whichever is earlier.
